Three of the most common are the T1, T3, and T5 forms.

These forms tell the CRA about your income, trust distributions, and investment earnings.

Knowing what each form is and how it works helps you complete your taxes accurately and claim the correct amounts.

Each of these forms has an official purpose and must be included or referenced when you do your tax return.

What is the CRA T1 tax form?

The CRA T1 Income Tax and Benefit Return form is the main tax form individuals in Canada use to file their annual personal income tax return. It is officially called the Income Tax and Benefit Return.

This form summarizes all the income you earned in the year, calculates your federal and provincial tax, and determines whether you owe money or will receive a refund.

The T1 form covers income from many sources, deductions, credits, and other adjustments.

It shows your total income, net income, taxable income, tax payable, and any benefits like the GST/HST credit or Canada Child Benefit you may be eligible for.

You report amounts from various tax slips, like T4, T5, and T3, on specific lines of your T1 return.

Most Canadians must file a T1 return every year. The standard filing deadline is April 30 for tax owed, or June 15 if you or your spouse or common‑law partner are self‑employed (with any balance owing still due by April 30).

What is the T5 Statement of Investment Income?

The T5 slip is officially called the Statement of Investment Income. It is used to report income from investments you earned during the tax year.

Bank accounts, investment firms, and other payers issue this slip if you received investment income like interest, dividends, or certain other amounts.

The T5 slip shows amounts such as interest from savings, dividend income from stocks, and other investment income paid to you.

If you earn more than $50 in interest or dividends from a financial institution, that institution generally must issue you a T5 slip.

You take the amounts shown on your T5 and enter them on the corresponding lines in the income section of your T1 return.

For people who need to issue multiple T5 slips (for example, a business or trust paying investment income to multiple people), there is also a T5 Summary form that totals all the T5 slips you prepare.

What is the T3 Statement of Trust Income Allocations and Designations?

The T3 form is officially the Statement of Trust Income Allocations and Designations. Trustees or administrators issue it when income from a trust is allocated to beneficiaries.

Trust income can include interest, dividends, capital gains, or other income that a trust earns.

A trust is a legal arrangement where one party (the trustee) holds property or assets for other people (beneficiaries). Trusts can be family trusts, mutual funds held outside registered accounts, estates, or other entities.

The T3 slip reports how much of the trust’s income or designated amounts beneficiaries must declare on their T1 return.

Like the T5, if you receive trust income shown on a T3 slip, you must report these amounts on your T1 return.

They can include several types of income, such as interest, dividends, capital gains, foreign income, and some return of capital amounts.

How do these forms work together on your tax return?

When the tax season arrives, you gather tax slips like T5 and T3 as part of your documentation. These slips show income you received during the tax year.

You take the figures from these slips and enter them into the designated spots on your T1 Income Tax and Benefit Return.

For example:

Include your investment income from a T5 on the income lines of your T1.

Include trust distribution income from a T3 on the appropriate section of your T1.

The T1 then calculates your total tax obligation or refund. If you fail to include income shown on these tax slips, the CRA’s matching system often flags the discrepancy and may reassess your return.

Do you need all these forms to file taxes?

If you earned employment income, investment income, or trust income, then yes. Your CRA T1 return is the form you submit to the CRA.

Slips like the T5 and T3 are not filed on their own. Instead, they support your T1 return by showing income you received during the tax year.

Most payers provide these slips by the end of February or March after the tax year.

If you do not receive a slip, you can request a copy from the issuer or view it through CRA My Account, where many slips are available once submitted by the issuer to the CRA.

What happens if a slip is missing?

If you do not receive a T5 or T3 slip before filing, you can estimate the income amount based on your records and include it on your T1.

You should attach a note explaining your estimate and then update your return later once you have the official slip.

Filing your return accurately with all income slips helps ensure that the CRA assesses your taxes correctly and that you receive the tax credits or refunds you are entitled to.

How can you access these forms and slips?

You can download current tax forms, including the T1, T3, T5, and guides, from the official CRA Forms and Publications page. You can also view slips issued to you online through CRA My Account after they are submitted by your payer.

Filing taxes in Canada starts with understanding key Canada Revenue Agency (CRA) tax forms and slips.
